The U.S. Government requires proof of right to work in the United States. Please bring the required documents listed on the form to your HR Representative.
Life Insurance Beneficiary Designation
This form allows you to designate beneficiaries for your basic life insurance, optional life insurance, accidental death and dismemberment, and business travel accident plans.
Relocation Agreement (If Applicable)
Barnes Group Inc. provides assistance to full-time salaried employees as well as exempt experienced new hires who are required to relocate on a permanent basis because of job responsibilities. Please fill out this form if you qualify for this benefit.

W-4 Federal Income Tax Withholding
This form is required so the appropriate amount of personal federal income tax can be withheld from your wages.

State Income Tax Withholding

Please choose the state or states that apply to you and fill out the state form so the appropriate amount of state income tax can be withheld from your wages. These forms
are for personal state income tax withholding only and do not change your federal withholding allowances.

PA does not have a state withholding form. A flat percentage is applied to determine the appropriate level of state income tax to withhold from earnings.
